The social service of the Klinik Dr. Baumstark advises patients and their relatives on all pending decisions concerning the organisation and further care after their stay in hospital. The discharge management also clarifies socio-legal issues for you. In addition, you will receive support with reintegration into working life or with discharge from the hospital as well as aftercare.
Patients do not only want competent and comprehensive care during rehab. They also want to be prepared for the time after their stay in the clinic, not least to further consolidate their state of health and to know how they can best cope with the everyday life ahead.
In preparation for discharge, a social service is available at the Klinik Dr. Baumstark to advise you. Our occupational therapy department is also the right contact when it comes to procuring aids. To ensure and improve the success of rehabilitation, aftercare measures can be recommended.
The training therapy rehabilitation follow-up care
The 39 training therapy units offered by the Deutsche Rentenversicherung must begin within three months after rehabilitation and be completed in twelve months.
